This privacy policy applies to the Debt Dismissal online hospital financial assistance application. We reserve the right to amend this notice at any time.
ScriptSourcing, LLC acts as a data controller with respect to the collection, use, and processing of individually identifiable information submitted in applications (“Personal Information”) in connection with Debt Dismissal.
For purposes of this privacy policy, “Personal Information” is information that you provide to us that may be used to identify you, including, but not limited to, your full name, physical address, email address, phone number, social security number, and information about your household. If we collect sensitive information, we will encrypt the transmission of that information using secure socket layer technology (SSL).
We utilize administrative, technical, and physical safeguards to protect your information that comply with applicable law and federal standards.
We may use or disclose the information we collect to fulfill the purpose for which you provided it, or we collected it, such as to submit the financial assistance application to the hospital network on your behalf. You may opt out of at any time.
We will not sell or rent this information to anyone, nor will we share this with any third party outside of our organization.

Data Security & Protecting PHI
Data security and protecting your PHI is of the highest priority. All data is encrypted and delivered securely to the appropriate processing endpoints.
Debt Dismissal uses a managed server hosting infrastructure that is fully audited and SOC 2, HIPAA, and HITECH certified. Hosting operations are routinely and systematically inspected. The focus is particularly on control objectives in the areas of organizational structure, governance, administration, physical/environmental regulations, and physical/logical security. All files are secured in transit with no less than 128-bit encryption and stored at rest using AES 256-bit encryption.
The Debt Dismissal Online Financial Assistance Platform includes: A Fully Managed Firewall Appliance, Security Analysis, On-Demand Updates and Patches, 100% SLA (Service Level Agreement), Encrypted Backup, Storage, & VPN, Fully Managed Daily Backups, Log Inspection System, and signed/executed BAA with business partners.
The platform also employs the Fully Managed Trend Micro® Deep Security Suite that includes: A Deep Security Anti Malware Package, Deep Security Network Security Package, & Deep Security System Security.
Our Cloud Platform includes: Cloud Backups Daily backup with 30 days retention, Disk level restore, as well as Onsite and Offsite backups.

For the period covering the 12 months preceding your request, you may request that we disclose to you the categories and specific pieces of information collected about you, the categories of sources from which we collected that information, and the purposes for which your information was collected.
You may also request that we delete information we collected from you.
